How to fill out the Electronic Participation Forms

1) You will need your students ID number (If you need the ID number please call the school office.
2) Students and Parents should do this together so each can provide necessary signatures.
3) Visit rankone.com
4) Select – For Parents
5) Select On Line Form (Go To Forms)
6) Select your State (Maine)
7) Select Mount Desert (This will take you to these instruction and the Parent Portal (Click Here)
8) This is where you log into your previous account or if your new to rank one you can create New Account (There are benefits to having an account as is allows much better communication – but you may just continue as a guest)
9) IF you create an account you will register and proceed – If you continue as a guest you will to on line forms.
10) Put your student first name, last name Student ID and select the School Attending. Be sure to add all of your students to your account.
11) Complete Forms as requested. Please read policies and sign.
12) Be sure to complete both the Extra Curricular Form and the Health Insurance Form. Your health insurance information must be filled out and a copy of your insurance card uploaded.
13) Physical Information: If your student has had a recent physical please have the Dr’s office submit it to the school.
14) IF your child is green – they are eligible to participate.
15) Please give 24 hours for data to be accepted.
